Tretinoin

- What it is
- Tretinoin is a prescription-only retinoid—the active form of vitamin A that directly binds to retinoic acid receptors in skin cells. It is the gold-standard retinoid in dermatology and requires a physician prescription.
- What it does
- Tretinoin accelerates epidermal cell turnover, increases collagen and elastin synthesis, and normalizes sebum production. It reduces fine lines, improves skin texture and tone, fades hyperpigmentation and post-inflammatory erythema, and is highly effective for acne and photoaging. Results typically appear within 8–12 weeks with consistent use.
- The evidence
- Strong clinical evidence supports tretinoin for photoaging, acne, and hyperpigmentation; decades of dermatological use and multiple RCTs confirm efficacy and safety at appropriate concentrations (0.025–0.1%). It is the benchmark against which other retinoids are measured.
- Best for
- Photoaging, fine lines, acne, hyperpigmentation, and uneven texture; suitable for most skin types when dosed and introduced gradually.
- Pairs well with
- Sunscreen (essential), moisturizers, niacinamide, azelaic acid, vitamin C (in separate AM/PM routines), and hydrating serums to buffer irritation.
- Use cautiously with
- Other potentially irritating actives (benzoyl peroxide, high-dose AHAs/BHAs, vitamin C at high pH) during initial weeks; avoid with other retinoids; use cautiously with topical antibiotics that may increase photosensitivity.
- Cautions
- Prescription-only; causes retinization (redness, peeling, sensitivity) for 4–8 weeks; increases sun sensitivity—daily SPF 30+ is mandatory. Contraindicated in pregnancy and breastfeeding. Start low (0.025%) and titrate slowly; not for sensitive or compromised skin barriers without medical supervision.
