Biostimulators

Poly-L-Lactic Acid

Injectable biostimulator that triggers collagen production over months.

Also known as: PLLA, polylactic acid, Sculptra, Sculptra Aesthetic

What it is
Poly-L-lactic acid is a biodegradable synthetic polymer derived from renewable resources like corn. When injected into the dermis, it acts as a biostimulator—not a filler—gradually breaking down while stimulating the body's own fibroblasts to produce new collagen.
What it does
PLLA works by triggering a mild inflammatory response that recruits fibroblasts to synthesize collagen over 4–12 weeks post-injection. This gradual collagen deposition restores volume, improves skin texture, and addresses age-related loss of elasticity. Results are subtle and progressive, peaking around 3–6 months and lasting 2+ years as the polymer is metabolized and collagen remodels.
The evidence
Strong clinical evidence supports PLLA for stimulating collagen deposition and improving skin laxity and volume loss; multiple randomized controlled trials and long-term follow-up studies confirm efficacy and safety for facial rejuvenation and lipoatrophy correction.
Best for
Age-related volume loss, skin laxity, and textural concerns in patients seeking gradual, natural-looking results; particularly effective for hollow cheeks, temples, and jawline definition.
Pairs well with
Hyaluronic acid fillers (for immediate volume while PLLA works), retinoids, vitamin C, and sunscreen to support collagen remodeling; can be combined with microneedling or radiofrequency for enhanced biostimulation.
Use cautiously with
Avoid injecting into areas prone to keloid or hypertrophic scarring; use caution with active inflammation or infection; do not combine with other biostimulators in the same session without clinical justification.
Cautions
Nodule formation is possible if improper injection technique or reconstitution is used; requires adequate hydration post-treatment; not recommended during pregnancy or active skin infections; results are delayed, requiring patient education on realistic timelines.
