Azelaic Acid

Antibacterial, anti-inflammatory dicarboxylic acid for rosacea and acne.

What it is
Azelaic acid is a naturally occurring saturated dicarboxylic acid found in grains like barley and rye. It's a lipophilic compound that penetrates the skin to target bacteria and inflammatory pathways without the irritation profile of stronger exfoliants.
What it does
Azelaic acid works through multiple mechanisms: it normalizes keratinization (gentle exfoliation), inhibits bacterial growth (especially P. acnes and Malassezia), reduces inflammatory mediators, and suppresses melanin production in melanocytes. This makes it effective for acne, rosacea, post-inflammatory hyperpigmentation, and melasma without aggressive peeling.
The evidence
Strong clinical evidence supports its use for rosacea (FDA-approved topical), moderate-to-severe acne, and post-inflammatory hyperpigmentation. Multiple RCTs demonstrate efficacy comparable to benzoyl peroxide and tretinoin, with emerging evidence for melasma and skin barrier repair in sensitive individuals.
Best for
Rosacea, acne-prone and sensitive skin, post-inflammatory hyperpigmentation, and melasma; particularly valuable for patients who cannot tolerate retinoids or stronger actives.
Pairs well with
Niacinamide, ceramides, hyaluronic acid, gentle hydrating serums; can layer with retinoids or vitamin C at different times of day.
Use cautiously with
No major conflicts. May be used cautiously with other exfoliants (AHAs, BHAs) to avoid over-exfoliation; space application times if combining.
Cautions
Initial mild irritation, tingling, or dryness is common; pregnancy category B (generally considered safe, but consult provider). Avoid if allergic to grains. May cause temporary redness in rosacea-prone skin before improvement.
